Make a Complaint

Please only complete this form if you would like to submit a complaint to the practice. This form is not to be used for general enquiries. Thank you. 

We make every effort to give the best service possible to everyone who attends our practice. However, we are aware that things can go wrong resulting in a patient feeling that they have a cause to make a complaint. We hope that most problems can be resolved quickly and easily at the time they first arise, however if this is not possible and you wish to make a complaint, the practice has an in-house concerns/complaints procedure.

Please contact the Practice Business Manager, Laura Armytage-Cornelius in writing by completing the Concerns Form. We will acknowledge and investigate your concern quickly and advise you of the findings.

The Practice Manager will aim to investigate your complaint within 30 working days of the date in which your complaint was first submitted.

During this period, we aim to:

  • Find out what happened and what went wrong.
  • Make it possible for you to discuss the problem with those concerned if you would like this.         
  • Make sure that you receive an apology where it is appropriate.
  • Identify what we can do to make sure this problem does not happen again.

We are aware that some patients may choose to post suggestions/recommendations for the surgery and the services we offer on Social Media platforms. Please note, whilst the surgery may have a response, we will not reply in such forums.

If you have a complaint, please address it directly to the surgery via the form below, where it can be properly investigated and addressed.
